Many students will be juggling after-school activities and sports schedules and it can be challenging to find a good time for piano lessons. Therefore, we are asking all current students to complete a survey to help us with scheduling for the new school year.
Current Start time
Choose the start time of the first student if more than one student in the family.
Check the box with the total number of minutes. Example: 2 students take 30 minute lessons would check the box for 60 minutes.

Please check ALL of your available start times for each day. If your first available start time falls between 15 minutes, check the time that is the EARLIEST that you can get to lessons.
