Weekly Lunch Menu
Check a menu item each day for your child's lunch choice. Please bring lunch and snacks if your child has food allergies or if you do not want any of the menu items for your child. Bottled water will be provided with meals and during breaks. However, children are welcome to bring their own water bottle as well. Snacks are individually packaged. Due to COVID precautions, sharing is prohibited.
Tuition rates are discounted for those bringing their own lunches.

FOOD POLICY
The staff will practice safe food handling procedures and will exercise caution when serving food. Parents agree that Denver Academy of Musical Arts and its staff are not responsible for any food born illness derived from the food served during camp.
Full tuition rates based on the number of meals we provide:

EMERGENCY CONTACT/RELEASE POLICY
In the event Parents and Secondary Contact can not be reached, the Emergency Contact has the Parents' permission to act as a temporary guardian in case of emergency. If neither Parents nor the Secondary Contact are able to pick the child up from camp, Parents give permission for their child to be released to the person(s) listed in the is section. The staff will check ID's and will not release the child if the ID does not match the name listed. NO EXCEPTIONS!
Medical Info
Parents may voluntarily provide doctor and insurance information to staff to forward to emergency medical personnel in case of emergency.
MEDICAL AND MEDICAL EMERGENCY POLICY/
Denver Academy of Musical Arts does not have medical staff onsite and therefore, the staff will not administer any medications, prescription or otherwise, while children are in their care. Parents must make arrangements for their child to receive medication if necessary.
Children and staff who do not feel well, should not attend camp. Upon arrival each morning, temperatures will be taken by digital thermometer. Children and/or staff with a fever (above 98.6) will not be permitted to attend camp. Every attempt will be made to allow the child the opportunity to make up the day another week.
Activities during camp pose minimal risk of physical harm, however in the event of an emergency, the staff will call 911 and attempt to contact parents and/or the emergency contact person. The staff will inform emergency medical personnel of the preferred choice of hospital, doctor and insurance information if provided; however, Denver Academy of Musical Arts, its staff and associates shall not be responsible for where the child will be treated. Parents shall be responsible for financial obligations related to their child's medical care including, but not limited to, ambulatory service and emergency room fees.
Rules and Discipline Info
Teachers and staff encourage creativity, collaboration and play. In order to create a safe space, there are rules in place to ensure all participants have the best possible experience. Teachers and staff will go over the rules each day with the group and make sure that the rules are clearly understood.
Children are expected to follow the rules as outlined:
1. Respect your teacher, the staff and other students.

Wait your turn to speak.

Keep your hands to yourself.

Use helpful language.
2. Respect the property, equipment. and instruments.

Wait until given permission to use the equipment and instruments.
-
Use the equipment and instruments as intended.
-
Handle equipment and instruments with care.
3. Wait until instructed before starting a task.
4. Activities are not mandatory. Anyone who is not comfortable participating in an activity may sit quietly and observe.
5. Talk with your teacher or another staff member if you have any questions or concerns.
Students who choose not to follow the rules will have the following repercussions:

1st offense -verbal warning from teacher

2nd offense- student will need to sit away from the group for 10 minutes.

3rd offense- student will have a private conversation with a teacher and will need to sit away from the group for 20 minutes and parents will be notified.

4th offense- student will be dismissed for the day. Parents will be notified to pick the student up.
Each day is a clean slate but if a parent is called to pick up a student on more than one day, the student will no longer be allowed to return for the remainder of the camp and there will be no refunds given.
RULES AND DISCIPLINE POLICY
Regarding the rules as outlined in the section "Rules and Discipline Info" In the event that neither parent can be reached, the Emergency Contact person will be notified if the student has 3 or more offenses in a given day. Parents agree that if their child must be dismissed, the child will not be allowed to attend a make-up day and there will be no refunds for the time missed. If the child is dismissed more than once during the week, he/she will not be allowed to attend the remaining days of the camp and no refund or make-up days will be granted. Parents are responsible for the cost of any damages to property and/or equipment owned by Denver Academy of Musical Arts and its owners incurred because of their child's negligence.
Cancellation Policy
A $100 nonrefundable deposit is required to hold a spot at the time of registration. The deposit will be applied towards the remaining balance. The balance is due May 15, 2023. If payment is not made by May 15, 2023, we will no longer reserve a spot for your child on the registered date. If you need to cancel prior to June 1, 2023 and you have paid in full, we will refund the cost of camp minus the deposit. On or after June 1, 2023, if your child is unable to attend camp during the week that you reserved and you have paid in full, we will do our best to reschedule; however, placement is not guaranteed.
Liability Waiver
I hereby release and agree to hold Denver Academy of Musical Arts harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself or my child and/or property that may be caused by any act, or failure to act of Denver Academy of Musical Arts, or that may otherwise arise in any way in connection with any services received from Denver Academy of Musical Arts. I understand that this release discharges Denver Academy of Musical Arts from any liability or claim that I, my heirs, or any personal representatives may have against Denver Academy of Musical Arts with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Denver Academy of Musical Arts. This liability waiver and release extends to Denver Academy of Musical Arts, with all owners, partners, and employees.
By signing this form you acknowledge that you have read and agree to all the terms and conditions in this registration form and that the information you provided is true and accurate to the best of your knowledge.
